Call Glenwood Medical Center at 320.634.5157 to schedule.\nStudents going into 7th and 10th grade must get an athletic exam to ensure they are eligible to participate in sports per the Minnesota State High School League. \nGet your complete physical exam at Glenwood Medical Center on August 9 or August 10 between 2 and 4:30 p.m. with nurse practitioners\, including a functional movement screening with an athletic trainer. Call early to schedule. \n\nAllow 45-60 minutes for the complete athletic exam\nIncludes a functional movement screen (FMS)\, clinic exam\, hemoglobin test (if needed)\, and vaccinations.\nBring your completed Sports Qualifying Physical Examination Medical Eligibility Form (click to print) and insurance information\nFree gift while supplies last\n\nNew this year – Athletic Exam at Minnewaska Area High School on August 4.\nYou may schedule a basic athletic exam at MAHS (by calling Glenwood Medical Center at 320.634.5157) – Thursday\, August 4 between 12 and 5:30 pm. A functional movement screen with an athletic trainer is provided as well. Immunizations and lab tests are not available with this option\, nor is billing insurance. This is a self-pay option – $75.00 via credit card\, cash\, or a check made payable to GRHS.
