DESCRIPTION:Let’s get your 7th and 10th grade student athlete ready for school sports. \nAthletic exams on Tuesday\, July 25 or Wednesday\, July 26 with nurse practitioners include a functional movement screening with an athletic trainer. \n\nAllow 45-60 minutes for the complete athletic exam\nIncludes a clinic exam\, hemoglobin test (if needed)\, and vaccinations\nBring your completed Sports Qualifying Physical Examination Medical Eligibility Form.\nFree duffle while supplies last\nThese are not typically covered by health insurance.\n\nCall Glenwood Medical Center at 320.634.5157 for an appointment.
