Nurse Practitioner / Physician Assistant

The Advanced Practice Provider (APP) will provide comprehensive care to patients in both an outpatient clinic setting at Glenwood Medical Center and through daily inpatient rounding at Glacial Ridge Hospital. This dual role focuses on ensuring continuity of care from hospital admission to post-discharge follow-up.

Duties & Responsibilities

Inpatient Rounding

  • Conduct daily rounds on hospitalized patients to assess clinical status and adjust treatment plans.
  • Manage admissions and discharges, including writing orders and completing discharge summaries.
  • Collaborate with the attending physician and multidisciplinary teams to coordinate complex care.

Outpatient Clinic

  • Perform comprehensive physical examinations and health histories for new and returning patients.
  • Diagnose and treat acute and chronic conditions (e.g., diabetes, hypertension, or acute injuries).
  • Order and interpret diagnostic tests, including labs, X-rays, and EKGs.
  • Refer patients to specialty care/services as needed.
  • Round on patients in the nursing home.
  • Collaborate with mentoring physician to coordinate care per Rural Health Clinic guidelines.

Procedures & Care Coordination

  • Perform minor office or bedside procedures such as wound care, suturing, or injections.
  • Prescribe medications and therapeutic regimens in accordance with state and federal laws.
  • Educate patients and families on disease prevention and healthy lifestyle choices.

Qualifications

  • Education: Master’s or Doctoral degree from an accredited NP or PA program.
  • Licensure: Current state licensure as an APRN or PA; active DEA and state controlled substance registration.
  • Certifications: Board certification (e.g., FNP, AGACNP, or PA-C) and current BLS/ACLS certification.
  • Experience: 3-5 years of clinical experience is preferred, often with a specific focus on acute or specialty care for rounding duties.

  • Basic computer knowledge.

  • Clear and timely documentation in the EHR.
  • Strong focus on patient-centered care and patient satisfaction.

Typical Work Schedule

  • Full-Time: Up to 40 hours per week.
  • Rotations: May include on-call duties, weekend rounding or clinic rotations, or evening shifts as needed for hospital coverage. May be asked to work at the Brooten or Starbuck Medical Centers.

  • Must be able to perform the physical requirements of the position.
