Complete Athletic Exam

Let’s get your 7th and 10th grade student athlete ready for school sports.
Athletic exams on Tuesday, July 25 or Wednesday, July 26 with Jessica Wilson, CNP or Micala Danielson, CNP include a functional movement screening with Athletic Trainer Kelly Shaner.
- Allow 45-60 minutes for the complete athletic exam
- Includes a clinic exam, hemoglobin test (if needed), and vaccinations
- Bring your completed Sports Qualifying Physical Examination Medical Eligibility Form.
- Free duffle while supplies last
- These are not typically covered by health insurance.
