Why do you take a survey? You get surveyed so you can express your opinion and share your experience. Is one person’s opinion really that important? You bet it is. Your answers and comments help organizations improve, along with continuing to do what they are already doing great.
New, Short Survey Process
You will see a new, shorter survey process starting May 1 from Glacial Ridge Health System. You will receive the 10-12 question survey within 24-48 hours of your appointment. We truly want to know how your experience was because your opinion is valued. This real-time feedback will help us implement changes faster and improve the quality of care we provide you.
For surveys not completed within 24 hours, you will get an automated phone call reminder, and you may answer the survey via phone at that time if you wish. Patients without an email address will get a phone survey.
If you are a patient at one of our clinics, surgical department, emergency department, therapy services, laboratory, or radiology (x-ray), you will receive the new emailed survey and reminder call. Some patients are seen in multiple departments within a short amount of time, after responding to any of these surveys you will only receive an emailed survey every 180 days (or up to two times a year). If the survey is not responded to, you will get another one after your next visit.
For Hospital Patients and Homecare / Hospice Clients
Hospital patients and homecare / hospice clients will continue to receive a survey to return in the mail because CMS (Center of Medicare and Medicaid Services) mandates these two surveys. Whether you receive an emailed or mailed survey, know they are extremely important to us and we want your feedback.
Confidential
As with all of our surveys, an independent organization conducts the survey and compiles the results. All answers are completely anonymous. If you have more to say than what the survey asks, we welcome you to contact us or enter your contact information in the comments section so we can further discuss your experience and gain a better understanding of your needs.
